COVID-19 Pre-appointment Information

Thank you for scheduling an appointment with our office. Please read the following completely.

• Upon scheduling your appointment you will be asked to complete and submit the COVID-19 Patient Screening Form 2 business days prior to your appointment.
• Only the scheduled patient will be allowed entry into the office unless a parent/guardian or caregiver is required. **The parent/guardian or caregiver will be screened as well upon entry.
• Provided that you have remained COVID-19 symptom free, then we ask that you arrive on appointment day at least 10 minutes prior to your scheduled time. Upon arrival, please remain in your vehicle or outside the building and call our appointment check-in number: 905-829-6001
• We will indicate to you when we are ready to have you enter the building. At that time please put on a clean mask. If necessary, one will be provided to you at the doorway into the office.
• The wearing of face masks and the practice of safe distancing is required in the office at all times when not in the process of undergoing your dental or hygiene treatment.
• You will be greeted at the office door by a Sherwood Dental team member who will ask you to sanitize your hands, check that you are properly wearing your own mask or provide you with one, take your temperature with a touchless thermometer, and have you again fill out our health screening form.
• A team member will then escort you to your specific treatment room.
